Personality disorder treatment in Waxahachie & DFW, without the stigma

Personality disorders carry more stigma than almost any diagnosis in psychiatry, and much of it is out of date. Borderline personality disorder in particular has strong evidence for recovery. Most people improve substantially with the right therapy. Progressive Pathways Psychiatry in Waxahachie offers evaluation, medication support and therapy coordination.

Quick answer

Personality disorders are long-standing patterns of thinking, feeling and relating that cause real distress and difficulty in relationships. They are treatable, and borderline personality disorder responds particularly well to structured therapy such as DBT. We evaluate carefully and coordinate the care that works.

What a personality disorder is

A personality disorder describes a long-standing pattern of thinking, feeling and relating to others that differs markedly from what a person’s culture expects, is stable across situations and time, and causes genuine distress or difficulty in daily life.

The word “personality” is what makes the term land badly. It sounds like a judgment about who someone is rather than a description of a pattern that developed for reasons and can change. In practice these are patterns of emotional regulation and relating, and patterns are exactly the kind of thing that responds to treatment.

Borderline personality disorder specifically

BPD is the personality disorder most often seen in psychiatric practice, and it is characterized by intense emotional responses, unstable relationships, fear of abandonment, an unsteady sense of identity, impulsivity when distressed, and often self-harm or recurrent suicidal thoughts.

Two facts about it deserve much wider circulation.

The emotional intensity is real and measurable. People with BPD experience emotions more strongly, reach the peak faster, and take longer to return to baseline. That is a difference in how the system works, not a matter of overreacting.

And the long-term outlook is good. Follow-up studies tracking people over a decade or more find that the majority no longer meet criteria for the diagnosis, and structured therapy shortens that timeline considerably. BPD is one of the more hopeful diagnoses in psychiatry, which is almost the opposite of its reputation.

How treatment works

Structured therapy is the core. Dialectical behavior therapy has the strongest evidence, and mentalization-based therapy and schema therapy also have real support. What they share is structure, a focus on emotion regulation and relationships, and a clear framework for crises. We coordinate with programs across the metroplex.

Medication is a support, not the treatment. No drug treats a personality disorder. Targeted prescribing for co-occurring depression, anxiety or sleep difficulties can genuinely help. What does not help is accumulating four or five medications over years, and one useful thing a psychiatric review can do is simplify that.

Trauma-informed throughout. Many people with BPD have significant histories of trauma or invalidating environments. That shapes how care is delivered: predictability, transparency, choice, and no surprises.

Safety planning as routine. If self-harm or suicidal thoughts are part of the picture, we build a plan for them rather than treating each crisis as an emergency in isolation. Knowing what to do at 2am, and knowing you can say it out loud at the next appointment, is part of treatment.

When to reach out

If your relationships repeatedly follow the same painful pattern, if emotions arrive at an intensity that makes them hard to survive, if you self-harm, or if you have been given this diagnosis and never offered real treatment for it, book an appointment.

If you are in immediate danger, call or text 988, or call 911.

What the evidence actually says

  • Recovery is the expectation

Long-term follow-up studies of borderline personality disorder show that most people no longer meet diagnostic criteria after several years, particularly with proper treatment.

  • Therapy is the main treatment

DBT, mentalization-based therapy and schema therapy all have real evidence. Medication supports the plan rather than being the plan.

  • Medication has a limited role

No medication treats the disorder itself. Targeted prescribing for co-occurring depression, anxiety or sleep can help, and stacking several drugs rarely does.

  • The emotions are not exaggerated

People with BPD experience emotions more intensely, reach peak faster and return to baseline more slowly. That is a measurable difference, not a matter of drama.

  • Trauma is often part of it

Many people with BPD have significant trauma histories, and trauma-informed care is not optional. It changes how the whole treatment is delivered.

  • The label is not a life sentence

A diagnosis is a description of a pattern that responds to treatment. It is not a verdict on character, and it should never be used to justify worse care.

Questions people ask

Frequently asked questions

Is borderline personality disorder treatable?

Yes, and better than its reputation suggests. Long-term studies show most people with BPD experience substantial improvement over time, and structured therapies like DBT accelerate that considerably. It is one of the more hopeful stories in psychiatry, badly served by the stigma attached to the name.

What is DBT?

Dialectical behavior therapy is a structured program combining individual therapy, skills groups covering distress tolerance, emotion regulation and interpersonal effectiveness, and phone coaching between sessions. It was developed specifically for BPD and has the strongest evidence base of any treatment for it. We coordinate with DBT programs in the Dallas-Fort Worth area.

Is there medication for personality disorders?

No medication treats the disorder itself. Medication is used for co-occurring conditions (depression, anxiety, sleep problems), and it should be targeted and conservative. People with BPD are frequently prescribed several medications at once with limited benefit, and part of good care is simplifying that.

Why do some clinicians avoid this diagnosis?

Historically the label has been used dismissively, and some clinicians avoid naming it to protect patients from that treatment. The cost is that people go years without being offered the therapy that actually works for them. We would rather name it accurately and treat it properly.

What if I am self-harming or having suicidal thoughts?

Tell us. It is one of the things we most need to know, and it will not be met with alarm or judgment. We will build a safety plan with you and make sure you have crisis options between appointments. If you are in immediate danger, call or text 988, or call 911.

More than one condition at a time is the rule rather than the exception. Treating what sits alongside the main problem is usually what makes the difference.
